Does Not Include: Medical advice, diagnosis, prescriptions, or treatment.

Routine Visit
$99 per visit (20 -30 minutes)

Focused visit to assess one health concern, such as:

✓ Medication review/medical question

✓ Minor illnesses

✓ Routine checkup

Follow-up Visit
$65 per visit (15 minutes)

For patients previously seen at our clinic for the same concern, including:

✓ Review of results

✓ Treatment progress

✓ Prescription renewals

Annual Check-Up 
$150 per visit (45-60 minutes)

Comprehensive preventive health visit that includes:

✓ Review of your medical history

✓ Review of your family history

✓ Screening recommendations

✓ Annual bloodwork

✓ Discussion of your overall health and wellness goals

Well Baby Visit (Birth to 18 months)
$99 (30-45 minutes)

Preventive visits focused on growth, development, and age-appropriate guidance. Includes:

✓ Physical exam

✓ Growth measurements (weight, height, head circumference)

✓ Developmental screening

✓ Immunizations

✓ Recommended at: 2, 4, 6, 9, 12, and 18 months, at 2 years, then annually.

Well Child Visit (2-11 years)
$99 (30 minutes)

Visit focused on growth, development, and age-appropriate guidance. Includes:

✓ Physical exam

✓ Growth & developmental screening

✓ Immunizations

✓ Nutrition counseling

✓ School concerns

✓ Preventative care

Teen Wellness Visit (12-18 years)
$120 (45 minutes)

A comprehensive preventive health visit designed to support the physical, emotional, and social well-being of adolescents during an important stage of development.

This visit focuses on helping teens build healthy habits, identify concerns early, and provide a confidential space to ask questions about their health.

Women’s Health– Perimenopause & Menopause Care Program ($250)

Women’s health visits focus on concerns such as menopause and perimenopause, PCOS, contraception, menstrual changes, and other hormonal or reproductive health issues. These appointments provide time to discuss symptoms, review your medical history, and develop a personalized treatment plan.

Visit 1 (45-60 minutes)

Comprehensive assessment including symptom review, menstrual/reproductive history, medical and family history, cardiovascular and breast health assessment, bone health, sleep, mood, sexual health, hormone therapy eligibility, start a treatment plan, and plan for investigations if indicated.

This can be virtual or in person.

Ongoing Care: 30-minute menopause follow-up visits ($70) as needed.

Visit 2 (30 minutes)

Follow-up visit (4–8 weeks later) to review results, optimize treatment, adjust hormone therapy if appropriate, and establish a long-term plan.


Mental Health– Adult ADHD Assessment Program ($350)

Before your visit, you will be asked to complete the online intake forms and validated ADHD questionnaires.

Visit 1 (75 minutes)

Comprehensive assessment including developmental, educational, occupational and psychosocial history, DSM-5 review, CADDRA-informed assessment, screening for anxiety, depression, trauma, sleep and substance use, and differential diagnosis.

This can be virtual or in person.

Ongoing Care: 20- 30-minute ADHD medication follow-up visits ($70).

Visit 2 (45 minutes)

Follow-up visit (4–8 weeks later) to review results, treatment recommendations, medication discussion if appropriate.


Annual Membership (unlimited visits for a 12-month period)

Personalized primary care with an advanced approach to disease prevention & management.

Adult Membership ($800 +HST per year )
Children Membership ($500 +HST per year)

Patients enrolled in the annual membership may receive:

  • Priority scheduling for same-day or urgent appointments when clinically appropriate
  • Virtual consultations for time-sensitive needs
  • Support with acute illnesses, medication questions, and follow-up care
  • Personalized, continuous medical support
  • Urgent prescription refills when an in-person visit is not required

Our goal is to help patients access timely care, navigate health concerns efficiently, and minimize unnecessary delays.


Virtual vs In-Person Care

We can help with some health concerns through virtual care (video or phone), but some health issues are better treated in person. Your health and safety are very important to us.
If the Nurse Practitioner feels that your concern cannot be safely managed virtually, you may be advised to attend a clinic, urgent care, or the hospital.

Below is a list of concerns that are generally appropriate for virtual care, and those that are not.

Appropriate for Virtual Care

If you are experiencing any of the following, a virtual appointment may be suitable:

  • Medication reviews and follow-up prescriptions, including controlled substances, when clinically appropriate
  • Urinary tract infection
  • Sore throat (less than seven days, or already treated)
  • Skin problems, rash, or shingles (less than seven days)
  • Tick bite
  • Stomach ache without fever (less than two weeks)
  • Mild to moderate headache
  • Mild breathing difficulty

Please note: prescribing decisions are made by the NP based on clinical assessment and safety.

Not Appropriate for Virtual Care

For the following concerns, it is important to seek in-person or emergency medical care:
✖ Acute or severe abdominal pain
✖ Life- or limb-threatening conditions
✖ Chest pain
✖ Acute or worsening shortness of breath
✖ Extreme lethargy
✖ Signs of stroke (such as trouble speaking or weakness on one side of the body)
✖ Reduced level of consciousness
✖ Severe headache (sudden or “the worst headache ever”)
✖ New difficulty breathing (trouble speaking, walking, or breathing when lying flat)
✖ Severe stomach pain with nausea or vomiting
✖ Baby under three months with a fever
✖ Baby aged three to twelve months with a fever lasting three days or more
✖ Very high blood sugar (>20 mmol/L) with thirst, fatigue, or dizziness
✖ Sudden loss of vision
✖ Sudden paralysis or rapidly worsening weakness
