Who is this for? Individuals who would like to learn about the practice and or their chosen provider and determine whether Routine Health Medical is the right fit.

Includes: Introduction to the clinic, review of specific NP experience and approach to care, discussion of services, fees, and next steps.

Does Not Include: Medical advice, diagnosis, prescriptions, or treatment.

You have one new health concern that has not previously been assessed by our clinic. Intended for one primary health concern.

Focused visit to assess one health concern, such as:

✓ New symptom

✓ Medication review/medical question

✓ Minor illnesses

You have already been seen by one of our NPs and are returning to review one established health concern, test results, medication response, or a previously discussed care plan, such as:

✓ Review of results

✓ Treatment progress

✓ Prescription renewals

You are generally healthy and would like to review recommended health screening, prevention, immunizations and health goals based on your health history and individual needs.

You take more then 3 regular prescription medications; live with multiple or complex chronic health conditions; have an extensive medical history; have not had preventive health care for several years; or would like a comprehensive review of medications, screening and health priorities.

You are booking a routine health and development visit for a child or youth, including growth, development, nutrition, sleep, school, emotional well-being, immunizations and preventive health needs.

Preventive visits focused on growth, development, and age-appropriate guidance. Includes:

✓ Physical exam

✓ Growth measurements (weight, height, head circumference)

✓ Developmental screening

✓ Immunizations

✓ Recommended at: 2, 4, 6, 9, 12, and 18 months, at 2 years, then annually.

You have recently moved to Canada and this is your first comprehensive health assessment with our clinic. This visit can review your health history, medications, chronic conditions, immunizations, previous screening and current health priorities, and identify preventive care recommended in Canada.

You are temporarily visiting Canada and have a new or ongoing health concern that you would like assessed while you are here.

You are experiencing new or changing symptoms that may be related to perimenopause or menopause and would like a comprehensive assessment and discussion of treatment options.

You have already had a menopause or perimenopause assessment with one of our NPs and are returning to review symptoms, treatment response, test results, medications or your existing care plan.

You have a new concern related to menstrual health, contraception, sexual or reproductive health, vaginal or vulvar symptoms, pelvic symptoms, fertility planning or another related concern.

One of our NPs has already assessed this concern and you are returning to review one established issue, results, medication response or your treatment plan.

You are due for routine cervical cancer screening and would like to book a trauma informed in-person screening appointment.

You would like an initial assessment of a mental health concern such as anxiety, depression, changes in mood, stress, sleep difficulties, difficulty coping, or other changes in emotional well-being.

You have already completed a mental health assessment with one of our NPs and are returning to review symptoms, treatment response, medication, test results or your existing mental health care plan.

You are seeking an assessment for possible adult ADHD. Additional questionnaires, records, collateral information or follow-up assessment may be required before a diagnosis or treatment plan can be established.

You have previously been diagnosed with ADHD by another qualified health-care provider and would like to establish ongoing ADHD care with Routine Health Medical.

Adult Membership ($800 +HST per year)

Children Membership ($500 +HST per year)

Patients enrolled in the annual membership may receive:

* Priority scheduling for same-day or urgent appointments when clinically appropriate

* Virtual consulations for time-sensitive needs

* Support with acute illness, medicating questions and follow-up care

* Personalized, continous medical support
